Naz Wellness Group - Home Monitoring Program
NazWellness group is comprised for a network of 14 pharmacies, and the groups founders created the Home Monitoring program to simply provide a vital service to the individuals that use our pharmacies. The Home Monitoring nursing service is a free program. We currently service 800 individuals in our daily Home Monitoring Program (primarily seniors and mental health) and our staff of over 60 LPNs provide the community service. This is a true collaboration with clinical pharmacists and nurses.
Our nurses work in the community on both AM and PM shifts allowing service to occur throughout the day from 0630 to 2230. Our pharmacies are located in most major cities in the lowermainland allowing us to service from West Vancouver to Coquitlam and in the south from Detla to Abbotsford.
We have deep working partnerships with units and teams with hospitals and community health centers where our referrals stem from. But we also intake individuals from family requests in conjunction with their general/nurse practitioners once it has been identified that there is a need for our support.
In terms of the support we provide for seniors we are able to complete the following during our visits:
- Patients unable to manage their medications, due to change in health status (e.g. stroke, heart attack, Alzheimer’s, etc.).
- Patients with physical impairments (mobility, vision) and/or cognitive barriers (mental health, addictions).
- Patients with a history of medication related hospital visits of medication administration errors.
- Patients with multiple chronic conditions or complex medications regimens.
- Patients with non-adherent with their medications therapy
- Patients requiring additional teaching or monitoring (e.g. insulin administration, inhalers, blood glucose meters, nebulizers, etc.)
- Diabetic treatment can get quite complex so we are able to help with administering and adjusting daily insulin doses
- Administering IM injections (i.e. Vitamins or long acting injections)
- Administering eye drops, creams, inhalers, and nebulizers
- To be the daily eyes and ears for someone that is otherwise somewhat isolated
- Reporting of changes in behaviors or any environmental concerns
- Our central team works with families and case workers to ensure any changes a communicated
